Long Term Care - Asset Discovery Investigation (LTC-ADI) Unit reviews applications and redeterminations of individuals requesting and receiving Long Term Supports and Services (LTSS) who reside in a nursing home, reside in a supportive living program, or receive Department on Aging (DOA) Home and Community Based Services (HCBS) waiver services when the information reported meets referral criteria, is questionable or unexplained.


The Medical Field Operation (MFO) Offices and Family and Community Resource Centers (FCRC) must refer applications and redeterminations to the LTC-ADI Unit that meets the referral criteria.


HFS has revised the referral criteria for the LTC-ADI Unit. A referral is no longer required for individuals who have a reverse mortgage, loans, or a home equity line of credit. Additionally, the total transfer amount has also increased from $10,000 to $25,000.


If the case meets the specific criteria after reviewing the application, data received from the Asset Verification System (AVS), the HFS 3654 form, the redetermination form, or any verifications provided,  the case should be referred to the LTC-ADI Unit.


All applicable documentation should be uploaded to the electronic case record (ECR) in IES. If the case meets the below criteria, refer to the LTC-ADI Unit using the revised form HFS 3654A LTC-ADI Referral Form (PDF) when any of the following criteria are met:

  • Transfers totaling $25,000 or more during the 5 year look back period
  • Evidence of estate planning, including but not limited to:
    • Trusts
    • Annuity
    • Promissory Notes
      • Contract for Deed
      • Caregiver Contracts
  • Consultation with a financial planner for estate planning purposes
  • Any other reason the caseworker deems appropriate (explanation required)

Referrals to the LTC-ADI Unit should be initiated on the Living Arrangement page in IES. See wizard IDHS OneNet: Record Living Arrangements for more details. Additionally, the completed 3654A form should be sent to the LTC-ADI Unit by email to HFS.OIG.LTC-ADI@illinois.gov. Include the type of referral and the case action (REDE, intake, reopen) in the subject line. Example below.
