Better Birth Outcomes - Comprehensive

Better Birth Outcomes Comprehensive (BBO-C) is a statewide program (outside the City of Chicago) that provides pregnant, and parenting clients with direct, comprehensive, 1:1 nursing assessments and assistance to connect and engage with desired services and understand the importance of a medical home.

Better Birth Outcomes - Navigation

BBO Navigation (BBO-N) is a program in the City of Chicago that helps pregnant and parenting clients understand the importance of a medical home and connect them to a myriad of medical, social, and other benefits and supports available in their community. Through in-person visits, the Navigator collaborates with families to identify and mitigate barriers to accessing desired supports and services.

High Risk Family Case Management - Pilot Program

High Risk Family Case Management (HRFCM) is pilot program that provides nursing assessment, intervention, and service coordination to improve the health, social, educational, and developmental needs of high-risk pregnant & postpartum individuals and/or their high-risk infants throughout pregnancy and the first one year after birth. Families requiring services beyond the first year may be authorized with Departmental approval.
