• To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire and Grant application. The Grant Application is to be completed for every program/grant. See Grant Application Information and Instructions webpage for more detail.
• At the time of application, all Bureau grantees must submit for each grant program the following documents:
  • Grant Application (pdf)
  • Uniform Grant Budget
  • Grant Application Program Narrative
  • CSA Budget/Tracking System and Registration Information
  • Conflict of interest
• The completed documents are to be emailed by July 31, 2024 5:30pm CST to the Bureau email address DHS.BRIS@Illinois.gov and Agata.Fieske-Nesheiwat@Illinois.gov with the subject line stating:
  • Last 4 digits of CSFA 3403
  • Provider Organization Name

For example, 3403 Jane's DV agency or 3403 HOPE agency

Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
