Grant Application Information

Only current grantees are eligible for funding under this program. To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire, and Grant application. The Grant Application is to be completed for every program/grant. See Grant Application Information and Instructions webpage for more detail.

At the time of application, all grantees must submit for each grant program the following documents:

• Grant Application (pdf)
• Uniform Grant Budget
• Conflict of Interest Disclosure
• CSA Budget/ Tracking System and Registration Information

The completed documents are to be emailed by June 18, 2024 to the email address DHS.BRIS@illinois.gov, Delyris.Martinez@illinois.gov  and Anna.Sawczuk-Raczkowska@illinois.gov with the subject line stating:

• "444-80-3057 Refugee Resettlement Program/Case Management"

Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
