GAF/CGAS Registration Data

Dear Community Providers,

We are pleased to announce the GAF/CGAS data will not be required as part of the consumer registration. There are complications around removing the associated fields so we are issuing the below work around to all community providers. If you prefer to continue to submit this information, the system will still accept the data.

Consumer Registration - GAF/CGAS Workaround

When submitting a Consumer Registration or Re-Registration, the system requires a GAF/CGAS score to be populated, along with some associated fields. If you are having an issue entering the GAF/CGAS score, or if the score is unknown, the Illinois Department of Human Services / Division of Mental Health (IDHS / DMH) is offering the workaround options below.

Workaround for ProviderConnect Direct Entry

On the 'Functional Impairment and Assessment Scores' tab, the Functional Scale Used will continue to be prepopulated with G - GAF or C - CGAS.

G - GAF
If the Functional Scale Used = G - GAF:
Enter 100 for the GAF score and select '00-Serious Impairment Criteria Not Met' for all eight of the dropdown fields associated with GAF.		 .
C - CGAS
If the Functional Scale Used = C - CGAS:
Enter 100 for the CGAS Score and select '00-Serious Impairment Criteria Not Met' for all five of the dropdown fields associated with CGAS.		 .

Workaround for Batch Submission

File Position 889 (Functional Scale Used): Enter the appropriate scale (G - GAF or C - CGAS).

File Position 890 - 892 (GAF/CGAS Score): Enter 100 for the score.

G- GAF

If Functional Scale Used = G - GAF:

Enter '00' (consumer does not meet serious impairment criteria) for all eight of the fields associated with GAF, which are found in the following file positions:

  1. 442 - 443 (Functioning -Adults Social Group/School)
  2. 444 - 445 (Functioning -Adults Employment)
  3. 446 - 447 (Functioning -Adults Financial)
  4. 448 - 449 (Functioning -Adults Community Living)
  5. 450 - 451 (Functioning -Adults Supportive Social)
  6. 452 - 453 (Functioning -Adults Daily Living Activity)
  7. 454 - 455 (Functioning -Adults Inappropriate or Dangerous Behavior)
  8. 456 - 457 (Functioning -Adults Previous Functional Impairment)

C - CGAS

If Functional Scale Used = C - CGAS:

Enter '00' (consumer does not meet serious impairment criteria) for all five of the fields associated with CGAS, which are found in the following file positions:

  1. 432 - 433 (Functioning -Children & Adolescents Self Care)
  2. 434 - 435 (Functioning -Children & Adolescents Community)
  3. 436 - 437 (Functioning -Children & Adolescents Social Relations)
  4. 438 - 439 (Functioning -Children & Adolescents Family Relations)
  5. 440 - 441 (Functioning -Children & Adolescents School)
