GA-Supplemental Nutrition Assistance Program -SNAP Job Placement/SNAP to Success (25-444-80-0707-02)

To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire, and Grant Application. The Grant Application is to be completed for every program/grant. See Grant Application Information and Instructions webpage for more detail.

Any changes made since FY24 to grantee mailing/billing address or Administrator/Executive/CEO name/email should be reported ASAP to IDHS to ensure proper information is reflected in the contract documents.

At the time of application, all Bureau grantees must submit for each grant program the following documents:

• Grant Application (pdf)
• CSA Budget/Tracking System and Registration Information
• Conflict of Interest Form
• All required documentation listed on the Notice of Funding Opportunity

The completed documents (minus the budget) are to be emailed by Friday, June 16, 2024 11:59pm CST to the agency contact email address, Kathy.L.Jackson@illinois.gov with subject line stating:

• • Last 4 digits of CSFA #0707-02
  • Provider Organization Name
  • For example, 0707-02 ABC Help Agency or 0707-02 SNAP Job Placement/SNAP to Success
• Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
