GA - Domestic Violence Health Services (25-444-80-3302)

Program Description - The Illinois Department of Human Services (IDHS) was awarded funding from the US Department of Health and Human Services Family Violence Prevention Services Act American Rescue Plan Act to focus on ensuring the safety, sustainability, and capacity building of domestic violence advocacy workforce during and after the COVID 19 pandemic. Expanding access to health and behavioral health services for survivors and their dependents is the primary focus of awards. Funds may be used to establish, expand, or maintain relationships with health and behavioral health service providers for service provision and/or be used to support personnel and related expenses to provide services within the domestic violence service provider agency. 

FY25 Application:  Current grantees will receive a continuation application packet via email.

  1. This continuation application is for Fiscal Year 2025 funding for Domestic Violence Health Services (CSFA #444-80-3302)
    1. Eligibility is limited to current Domestic Violence Health Services funded under CSFA # 24-444-80-3302. 
    2. Each applicant must be registered in the Illinois GATA Grantee Portal and must continue to be Illinois GATA Pre-Qualified.
    3. Applicant organizations must complete the FY25 ICQ before an award can be made. (Accessed through the Grantee Portal.)
    4. Applicant organizations must submit a complete FY25 Application Packet to DHS.DVSAHT@illinois.gov  by April 15, 2024 by 12:00pm. Include "CSFA# 444-80-3302" on the subject line.

A complete application packet includes:

  1. Uniform Application For Grant Assistance (pdf)
  2. FY 25 Program Plan
  3. Updated Contact Information
  4. Grantee Conflict of Interest Disclosure Form
  5. FY 25 budget must be entered into CSA by application due date.  Grantees will be notified of the grant award amount to enter into CSA.

The Bureau of Domestic Violence Sexual Assault and Human Trafficking reserves the right to request additional information from the applicant to assist in FY25 funding and programmatic decisions.
