GA - Special Education - EI Child & Family Connections (25-444-84-2880-01)

To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire and Grant Application. The Grant Application is to be completed for every program/grant.

At the time of application, all Bureau grantees must submit for each grant program the following documents:

• Grant Application (pdf)
• CSA Budgeting/Tracking System and Registration Information

The completed narrative documents must be emailed to DHS.EIFiscal@illinois.gov with the subject line including:

• 2880-01 (the last 4 digits of CSFA)
• Provider Organization Name - For example, 2880-01 Special Education - EI Child & Family Connections

Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
