To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire and Grant Application. The Grant Application is to be completed for every program/grant.
At the time of application, all Bureau grantees must submit for each grant program the following documents:
The completed narrative documents must be emailed by May 25, 2024 to DHS.HomeVisiting@Illinois.gov with the subject line including:
2900-01 (the last 4 digits of CSFA)
Provider Organization Name - For example, 2900-01 Special Education - EI Clearinghouse
Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
