GA - Parents Care and Share (25-444-84-2890)

To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire, and Grant Application. The Grant Application is to be completed for every program/grant.

FY25 Application: Eligibility is limited to current Parents Care and Share grantees funded under opportunity number 24-444-84-2890.  Current grantees will receive a continuation application packet via email. 

At the time of application, all Bureau grantees must submit for each grant program the following documents:

• Grant Application (pdf)
• Program Plan (from continuation application email)
• CSA Budgeting/Tracking System and Registration Information - Refer to the instructions in the continuation application email.
• Grantee Conflict of Interest Disclosure

The completed narrative documents must be emailed by May 10, 2024, to DHS.HomeVisiting@Illinois.gov  with the subject line including:

• 2890 (the last 4 digits of CSFA)
• Provider Organization Name - For example, 2890 HOPE Agency

Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
