To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire, and Grant Application. The Grant Application is to be completed for every program/grant.
FY25 Application: Eligibility is limited to current IDHS Home Visiting grantees funded under opportunity numbers 24-444-84-2888-01 and 24-444-84-2889-01. Current grantees will receive a continuation application packet via email.
At the time of application, all Bureau grantees must submit for each grant program the following documents:
The completed narrative documents must be emailed by May 3, 2024, to DHS.HomeVisiting@Illinois.gov with the subject line including:
- 2889-01 (the last 4 digits of CSFA and last 2 digits of opportunity number)
- Provider Organization Name - For example, 2889-01 HOPE Agency
Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
