GA-Responsible Parenting (25-444-84-2891)

To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire, and Grant Application. The Grant Application is to be completed for every program/grant. See Grant Application Information and Instructions webpage for more detail.

FY25 Application: Eligibility is limited to current Responsible Parenting grantees funded under opportunity number 24-444-84-2891.  Current grantees will receive a continuation application packet via email. 

At the time of application, all Bureau grantees must submit for each grant program the following documents:

• Grant Application (pdf)
• Program Plan (from continuation application email)
• CSA Budget/Tracking System and Registration Information - Refer to the instructions in the continuation application email.
• Grantee Conflict of Interest Disclosure

The completed documents are to be emailed by May 1, 2024, to the Bureau email address DHS.HomeVisiting@Illinois.gov  with the subject line stating:

• Last 4 digits of CSFA #2891
• Provider Organization Name- For example, 2891 HOPE Agency

Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information
