Targeted Hospital Sites:  Volumes of Front Door Referrals

Note:  FY22 referral totals represent two separate data sources.  July 1, 2021 - April 17, 2022 from DHS PASRR, and 4-18-22 - June 30, 2022 from Maximus.

* The Lake County referral volumes represent all eligible referrals for the program as consumer engagements are underway with current program structure.

** The McHenry Hospital and Huntley Hospital sites do not have Behavioral Health inpatient units.  Referrals from these sites are likely by crisis teams in Emergency Departments or medical floors who have or will refer that consumer for admission to the Woodstock Hospital site.  Thus the total volumes of the three should be considered as one probable "site" for engagement.
