Last week, the Division of Developmental Disabilities (DDD) sent out two communications that included information on the ongoing implementation of Electronic Visit Verification (EVV). All DDD communications can be found on the DDD Communications webpage. This communication is in follow up to those previous communications and is specifically geared towards the Live-In Caregiver Attestation Form.

As a reminder, if the Personal Support Worker (PSW) is not requesting the Live-In Caregiver exemption, there is no need to submit the Live-In Caregiver Attestation Form. As for those PSWs requesting the exemption, the Attestation Form must include all information in order to be accepted/approved. The Attestation Form must contain electronic or original signatures from the PSW and the Employer of Record (EOR). Writing the PSW or the EOR's name in italics in this space is not an acceptable signature. Additionally, the Attestation Form must be accompanied by the supporting documentation proving the PSW and individual's addresses. The only acceptable forms of supporting documentation are listed in the DDD Information Bulletin (IDHS: Live in Caregiver Exemption (state.il.us)). As a reminder, the Attestation Form itself includes a shorter list of acceptable supporting documentation; please use the supporting documentation list provided on the Information Bulletin linked above. Once you have fully completed the form and gathered all of the supporting documentation, you will submit it to your PSW Agency or ACES$, whichever entity from which you purchase services. DDD is hosting a training on the Attestation Form. Training information including the registration link is provided below.
