Requests for disbursement shall be submitted via the IL444-1961, Application for Funeral and Burial Assistance for the Murdered Children Program, including all supporting documentation, to the Bureau of LOTSS, Funeral and Burial Unit at the following address:

  • Springfield Illinois 62762

(F&B) The Funeral and Burial Unit shall promptly date stamp any request and supporting documentation upon receipt. The request shall then be screened for accuracy, completeness and supporting documentation.

The disbursement request should be processed, and once a determination is made, an IL444-1959, Funeral/Burial Notice of Decision should be issued manually by the Funeral and Burial Unit. The request for payment and all necessary documentation should be sent to the Bureau of Fiscal Operations for processing of the payment via the Illinois Comptroller.

If the claim cannot be processed for any reason, the Funeral and Burial Unit shall notify the submitting funeral establishment or cemetery authority of the necessary documentation needed to process the request for disbursement.

The Funeral and Burial Unit shall request additional documentation as needed by issuing an IL444-1962, Murdered Children Funeral and Burial Assistance Program - Request for Additional Information and monitor the 30-day timeframe.

Once approved for payment, the Funeral and Burial Unit will submit a claim to the Bureau of Fiscal Operations to have the payment processed.

(FCRC) No action is required by the FCRC. Inquiries from funeral establishments and/or cemetery authorities regarding the Murdered Children Funeral and Burial Assistance program should be submitted in writing by the funeral establishment and/or cemetery authority, to the Funeral and Burial Unit at DHS.FB@illinois.gov.

Inquiries from individuals regarding the Murdered Children Funeral and Burial Assistance program should be handled in the FCRC using published policy. Should the individual have questions relating to any payment, they should be directed to the funeral establishment or cemetery authority who submitted the request for payment for expenses on behalf of services provided to the child victim.
