Discharge Planning
Facility - Aftercare - Discharge
- Begins when a youth is committed to IDJJ
- Facility teams are responsible for identifying a host site.
- Prior to Aftercare Release the host site must be assessed by Aftercare for appropriateness.
- In the event a youth has no viable host site, facility refers to the Placement Resource Team.
- The Placement Resource Team identifies appropriate placement options for the youth.
- Upon commitment to IDJJ each youth is assessed for service needs (mental health, psychiatric, substance abuse, juvenile sex offender treatment, multi systemic therapy for emerging adults, mentoring, vocational and educational needs etc. )

Facility - Aftercare - Discharge
- Based off assessments each youth is referred to services/resources
- Referral's are monitored via multi disciplinary team meetings for the duration of the Aftercare Term
- Aftercare Teams are responsible for completing a 6 month review and 12 month review, 1.5 year review and or Early Discharge Review on each youth that identifies the discharge plan to include housing needs.
- Based on constant monitoring of youth's needs related to services/resources and housing, the Aftercare Teams utilize community agencies to assist the youth with housing supports for the youth.
