  • Choate DD: Medium security, ID only; males only
  • Chester: Maximum & Medium security, MI, males only, high risk, aggressive, elopement risk & other factors.

*Minimum Security Criteria: misdemeanor, low elopement risk, low risk for aggression, & males and females

Civil Patients Entering and Leaving a SOPH

Civil Patients

  1. Develop treatment goals and begin discharge planning process with patients
  2. Review treatment goals and progress. Continue to assess patients' needs and level of functioning
  3. Explore housing options and secure housing in the community
  4. Achieve treatment goals and finalize discharge plan
  5. Link patients to community providers for engagement and outpatient services.
  6. Discharge patients with confirmed aftercare appointment and medications.

Forensic Patients Entering and Leaving a SOPH

UST Patients

  1. Develop treatment and fitness restoration goals with patients.
  2. Review treatment goals and progress made toward fitness & stabilization
  3. Report patient fit to stand trial to the court once attain fitness.
  4. Remand patients back to the custody of the sheriff by the court to stand trial.
  5. The disposition of the defendant is determined by the court.

NGRI Patients

  1. Develop treatment goals with patients.
  2. Review treatment goals and progress made toward psychiatric stabilization
  3. Work with patients to mitigate risk factors and develop a solid plan for conditional release.
  4. Submit conditional release packet to the court for approval.
  5. Discharge patients on conditional release or once reach Thier date.
