To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire and Grant application. The Grant Application is to be completed for every program/grant. See Grant Application Information and Instructions webpage for more detail.
At the time of the application, all Bureau grantees must submit for each grant program the following documents:
Complete applications must be received by The Department at the specific location no later than 11:59PM Eastern Time, April 28, 2023.
The completed documents are to be emailed to the Bureau email address DHS.ServeIllinois.NOFO@illinois.gov with the subject line stating:
Last 4 digits of CSFA #2809-02
Provider Organization Name- for example, 2809-02 Jane's Agency or 2809-02 HOPE Agency
Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
