SNAP Management Evaluations (ME)
The Illinois Department of Human Services (DHS) is required by the USDA Food and Nutrition Service to assess SNAP (Supplemental Nutrition Assistance Program) access, customer service and civil rights issues. These assessments are completed by the Bureau of SNAP Integrity and are called SNAP Management Evaluations (MEs).
SNAP ME Survey
Part of the SNAP ME process includes sending customer surveys to applicants. Survey respondent's identity will not be revealed to anyone who works in the office, and the information collected will not impact their participation in any programs. Participation is completely voluntary. The Bureau of SNAP Integrity will send surveys by mail or email, if the applicant has indicated they will receive electronic notifications. The Bureau of SNAP Integrity will not request items such as social security number or case number through email.
Applicants who receive a survey and wish to complete the survey can return them by email to DHS.BSNAPI.ACE@illinois.gov or mail to:

Surveys can also be completed by phone by contacting the reviewer at the number listed on the survey.
