Uniform Grant Agreement
What is the Uniform Grant Agreement?
The Uniform Grant Agreement (UGA) contains standard contract language used for all agreements between the Illinois Department of Human Services (IDHS) and provider grantees receiving funds by the Department. It is a legally binding document to implement services consistent with the Program Plan and the Funding and Service Level indicated in the UGA attachments. These three documents outline the services, duties, and responsibilities of the provider in implementing the program under applicable state and federal regulations.
After successfully complying with the Notice of Funding Opportunity (NOFO) and all grant requirements, a provider receives a UGA to review. Providers should be sure to review the agreement before signing and returning the signature page to IDHS. The Department will also sign and return a copy of the executed Agreement to the provider.
As part of the UGA attachments, the Program Plan is referenced as an Exhibit. Any changes to the Program Plan during the contract period require prior approval from IDHS program staff. If the Program Plan does not adequately address program requirements, IDHS program staff will contact the provider and offer technical assistance to develop and complete the required information.
If the provider does not submit a completed Program Plan by the date specified by the Department, it may result in delaying the execution of the Agreement, termination of the Agreement, or a modification of the award decisions. Providers receive written notification of any actions taken by the Department to obtain a completed Program Plan.
An amendment may be required when there is a change in the terms of the UGA after it has been approved by IDHS. The changes to the Program Plan may include covered services, service delivery, deliverables, and the budget. The provider must contact IDHS program staff to discuss the proposed changes and to determine if an amendment is necessary. The provider prepares the details of the proposed amendment and supporting documentation for IDHS approval.
All amendments must be submitted to the Bureau administering the grant at least thirty (30) days prior to the proposed effective date and thirty (30) days before the end of the Agreement period (June 30). Processing time required by IDHS should be taken into consideration.
