Over Due Attestations
Thank you to all those providers who completed the attestation regarding residency agreements, lockable doors and modifications. Unfortunately there are some providers who have not yet completed the attestation. If you are one of those providers, please fill it out immediately or let us know if you have questions or issues by emailing DHS.HCBS@illinois.gov.
