WAG 03-11-05: Obtaining an SSN for a New Unit Member

PM 03-11-05.

  1. (Client) Requests addition of a person to the case, or Family Community Resource Center learns that a person who must be included in the case is living in the home.
  2. (FCRC) Ask the client to provide an SSN for the new member.
    1. If the client provides a verified SSN for the person, enter the SSN in Item 60 on the Form 552 when adding the person.
    2. If the client cannot provide a verified SSN, or the person does not have an SSN, tell the person that an application for an SSN is a condition of eligibility.

      NOTE: A newborn up to age 12 months can receive Medicaid only without having to provide or apply for an SSN. 

  3. (FCRC) Complete Requirement to Apply for Social Security Number Card (Form 3168) and place a copy in the case record.
  4. (FCRC) Give the person Form 3168 and Form 267 or Form 1721 and tell them to report to the local Social Security District Office. Tell the client they must give SSA proof of age, identity, and citizen/INS status.
  5. (FCRC) Allow the client 10 calendar days to return proof that an application for an SSN has been filed.
  6. (FCRC) Document in the case record the date proof of filing for an SSN was received in the case record.
  7. (FCRC) Tell the client to report the SSN when they get it.
