WAG 03-11-01-a: Enumeration at Birth (EAB)

PM 03-11-01-a.

  1. (Client) Applies for SSN for newborn while in hospital.
  2. (Client) Provides FCRC with one of the following forms of proof that an application for an SSN was made:
    • child's certified birth certificate indicating Enumeration at Birth (EAB) option,
    • copy of original birth certificate that shows the name of the hospital and an authorized hospital official, or
    • SSA 2853 that includes the child's name and signature of hospital official.
  3. (FCRC) Give the parent of a newborn until the first day of the 2nd calendar month after birth of child or discharge from hospital to apply for an SSN for the child. For medical only, the parent does not have to provide verification of application for an SSN until the child is age one if the mother was receiving Medicaid at the time of the child's birth.
  4. (FCRC) Accept certified original of EAB documents only.
