Attachment C - Service Provision Information

Attachment C must include at least the following information:

• Number of Unduplicated Clients Served (Adult, Children, Total)
• Number of Direct Service Hours (Adult, Children, Total)
• Describe ability to meet comprehensive needs of victims including housing, employment, health, mental health, substance use, parenting, childcare.
• Describe emergency shelter capacity (comprehensive only)
