WAG 06-10-04-g: Liaison with Provider

  1. (FCRC/All Kids Unit) Assign a person(s) to be the liaison with the MPE qualified providers.
  2. (Liaison) Gives provider the date and time of the application interview for the MPE client to apply for Cash and/or SNAP benefits at the FCRC.
  3. (Liaison) Tracks the status of the Healthy Start Applications (Form 3089).
  4. (Liaison) Takes follow-up calls from the provider and provides them with information, such as whether the appointment was kept.
  5. (Liaison) Gives the provider a new appointment date if the client did not keep the appointment and the provider calls within 45 days of the MPE decision date.
  6. (Provider) Tells the client about the new appointment.
