MR #22.07: Funeral and Burial Vendor and Reimbursement Claims Process

Summary:

Effective immediately, this manual release changes the process for Funeral and Burial Vendor and Reimbursement claims.
Family Community Resources Centers (FCRCs) will no longer process Funeral and Burial Vendor and/or Reimbursement claims.
All Funeral and Burial Vendor and Reimbursement claims will be processed by the Funeral and Burial Unit within the Bureau of Local Office Transactions and Support Services (LOTSS).
Funeral and Burial Vendor and Reimbursement claims should be forwarded immediately to the Funeral and Burial Unit at: DHS.FB@illinois.gov.
Vendors must continue to complete the IL444-0029, Funeral or Burial Claim form and claimants the IL444-0094, Funeral and Burial Reimbursement Claim form to be eligible to receive a reimbursement for services.
IDHS forms are being revised with this manual release.

Effectively immediately, all Funeral and Burial Vendor and Reimbursement claims are processed by LOTSS in the Funeral & Burial Unit. FCRCs will no longer process Funeral and Burial Vendor and/or Reimbursement claims.

Funeral and Burial Unit

All Funeral and Burial Vendor and Reimbursement Claims are now processed through the Funeral and Burial Unit in LOTSS.

FCRC

FCRCs will no longer process Funeral and Burial Vendor and/or Reimbursement claims. All claim requests received in an FCRC should, upon receipt, be date-stamped and forwarded to the Funeral and Burial Unit using the following e-mail address: DHS.FB@illinois.gov.

Forms

Vendors must still complete the IL444-0029, Funeral or Burial Claim form and claimants the IL444-0094, Funeral and Burial Reimbursement Claim form to be eligible for reimbursements through IDHS. Funeral and Burial forms are currently being revised and updated to reflect the changes.
