A. Program Description
The Faith-Based Housing Awareness and Assistance Program (HAAP) seeks to engage Faith-Based Grassroots Organizations (FBGO) located in neighborhoods severely impacted by high rates of COVID positivity and COVID related deaths, and disproportionately impacted communities to provide direct contact with community members to raise awareness of rental and utilities assistance programs available statewide.
The Housing Awareness and Assistance Program (HAAP) educates and provide application assistance to individuals that have been disproportionately impacted by the COVID-19 Pandemic and are in danger of eviction or have lost or in danger of losing utility services of available resources. The goals of the Housing Awareness and Assistance Program are to stabilize unemployed and under-employed low-income individuals and families in their existing homes and to provide trusted, culturally competent outreach, messaging and Application assistance to sustain thriving communities.
FBGOs are expected to:
- Have appropriate resources to provide culturally and community appropriate outreach in disproportionately impacted communities in a self-identified geographical area.
- Have a strong working knowledge of the target population's needs
- Be located in the Black disproportionately impacted communities to be served
- Be equipped with the resources to work cooperatively with other organizations who provide direct services, assistance, and make appropriate referrals to households
- Be knowledgeable of all existing rental and utilities assistance programs and eligibility requirements
- Be able to refer community members to the appropriate agency to complete the application and verification
- Provide advocacy on behalf of individuals to increase application approvals
- Provide information to community members about resources available for rental and utilities assistance and other IDHS resources
- Provide rental and utility Application assistance to community members by accepting and processing applications
- Track outreach activities in communities based on geography (zip codes)
- Produce and distribute IDHS approved collateral (flyers, business/palm cards, resource guide)
- The Provider will adhere to the IDHS program requirements including non- discrimination and community-wide access to Program Services.
IDHS requires the successful applicants to meet the following performance standards
- Outreach provided to at least 80% of the projected number of customers.
- Application assistance provided to at least 80% of the projected number of customers.
- The Provider will report deliverables and outcomes by the 10th of the month following the end of the month utilizing the web-based reporting system.
- Use Periodic Programmatic Reports (PPR) and Periodic Financial Report (PFR) forms to report status
- Respond timely to requests from IDHS for information
- Timely and accurate fiscal and program reports in format required by IDHS.
Housing Awareness and Assistance Program Priorities:
Special consideration will be given to proposals that represent the ability to serve the following identified priorities. Those priorities are as follows:

3. Payment Terms
The Illinois Department of Human Services (IDHS) payment policy complies with 2 CFR 200.302, 2 CFR 200.305, and 44 Ill. Admin. Code 7000.120 (GOMB Adoption of Supplemental Rules for Grant Payment Methods) and the and the Cash Management Improvement Act and the Treasury-State Agreement (TSA) default procedures codified at 31 CFR 205. IDHS Payments to grantees will be governed in accordance with the established criteria set forth in IDHS Administrative Directive 01.07.01.070 - Grant Payments. In accordance with IDHS Administrative Directive 01.07.01.070 - Grant Payments, grantees have the following payment options.

C. Eligibility Information
1. Eligible Applicants:
This funding Application is limited to those public or private, not-for-profit community/faith-based agencies subject to 26 U.S.C. 501 (c) 3 in good standing with the State of Illinois. Failure to provide the requested information as outlined herein to demonstrate these criteria are met will result in the application being removed from funding consideration.

b. Has current and active ties to the community being served
c. Associated with a fully functional church or place of worship
d. Has prior experience and knowledge of the target population
e. Demonstrates understanding in ethnic diversity of the Black community and the need for culturally appropriate services unique to this program
The FBGO should have experience working in the community they wish to serve, FBGO should demonstrate the ability to track data and produce outcome reports. The FBGO should be currently working with low-income populations in the provision of services or outreach.
Staff assigned to the Faith based Housing Awareness and Assistance Program should have the appropriate training and capability to provide culturally competent outreach and education to the target population.

State agencies are federally required to evaluate the risk posed by each applicant. Risk assessments are a pre-award requirement and include two components: fiscal and administrative and programmatic. Illinois uses an Internal Control Questionnaire (ICQ) to complete the fiscal and administrative component. Entities are required to access, complete and submit the ICQ through the Grantee Portal for the state fiscal year associated with the grant funding. The programmatic risk assessment is distributed by the state awarding agency as part of the application process. Both the ICQ and the programmatic risk assessment must be accepted by the state and approved before a grant award can be executed.
3. Cost Sharing or Matching
There is no cost-sharing requirement for this program
4. Indirect Cost Rate
Annually, each organization receiving an award from a State grant making agency is required to record an indirect cost rate election through the Grantee Portal or negotiate an indirect cost rate through the Indirect Cost Rate System. Indirect costs cannot be compensated without an election or a negotiated rate is one file with the State of Illinois.
The following elections for indirect costs to State and federal pass-through grants are available through the Grantee Portal:
A) Report a Federal Negotiated Indirect Cost Rate Agreement (NICRA);
B) Elect the de minimis rate of 10% of Modified Total Direct Costs; or
C) Elect not to charge indirect costs.
An indirect cost rate may be negotiated through the Indirect Cost Rate System.

The awardee shall make an election or negotiate a rate that all State agencies must accept unless specific federal or State program limitations, caps or supplanting issues apply.

2. Content and Form of Application Submission:
A. Submission Format, Location and Deadline
i) A Letter of Intent must be submitted by noon August 2, 2021. The Letter of Intent is NOT a pre-requisite to the grant application. However, IDHS needs the information provided from the Letter of Intent to prepare the application review process and invite your organization to the Technical Assistance workshop.
ii) Applicants must electronically submit the complete application including all required narratives and attachments in the prescribed order. Applications must be submitted electronically to Asia.Moore@ilinois.gov no later than 11:59p.m. on August 11, 2021 The application will be electronically time-stamped upon receipt. The Department will ONLY accept applications submitted by electronic mail. The subject line of the email MUST state: "22-444-80-2562-02 Faith-Based Housing Assistance Awareness Program" The electronic copy must be a complete single PDF file. Applications will NOT be accepted if received by fax machine, hard copy, disk, or thumb drive. Please refer to H. Mandatory Forms -- Required for All Agencies, i. Uniform Grant Budget - The proposed budget must be entered, signed and submitted in CSA and is required for the application to be considered complete. A copy of this signed and submitted budget must be included with the application.
3. Unique Entity Identifier/Dun and Bradstreet Universal Numbering System (DUNS) Number and System for Award Management (SAM)
Each applicant (unless the applicant is an individual or Federal or State awarding agency that is exempt from those requirements under 2 CFR § 25.110(b) or (c), or has an exception approved by the Federal or State awarding agency under 2 CFR § 25.110(d)) is required to:

E. Application Review Information
All competitive grant applications are subject to merit-based review. Review criteria is based upon the requirements set forth in the following:

As dictated by the authoritative references above, the following minimum review criteria apply:
- Need: Identification of stakeholders, facts and evidence that demonstrate the proposal supports the grant program purpose.
- Capacity: The ability of an entity to execute the grant project according to project requirements.
- Quality: The totality of features and characteristics of a service, project or product that indicated its ability to satisfy the requirements of the grant program.
- Other unique program criteria.

ALL applicants must demonstrate that they meet all requirements under this NOFO as described throughout. Applications that fail to meet the criteria described in the Eligible Applicants and Mandatory Requirements of the Applicants as identified in Section C Eligibility information will not be evaluated and considered for funding.
Grant Applications received will be reviewed by IDHS staff. The maximum possible score is 100 points.
||The Executive Summary (1-2 pages MAX) will serve as a stand-alone document for successful applicants that will be shared with various state-level stakeholders and others requesting a brief overview of each funded project. At a minimum, each of the following should be addressed in the summary:
- be a Faith-Based Grassroots Organization; AND
- have one or more physical offices located in the state of Illinois; AND
- be pre-qualified to apply for this funding notice through the Grant Accountability and Transparency Act (GATA) Grantee Portal.

- 1. Review and Selection Process
- As described in the Criteria section above, scoring will be on a 100-point scale. Scoring will not be the sole award criterion. The Department reserves the right to consider other factors such as: geographical distribution, ICQ and PRA results, demonstrated need, past performance as a state grantee, etc. While recommendations of the review panel will be a key factor in the funding decisions, the Department maintains final authority over funding decisions and considers the findings of the review panel to be non-binding recommendations. Any internal documentation used in scoring or awarding of grants shall not be considered public information.
- The Department reserves the right to negotiate with successful applicants to adjust award amounts, targets, etc. The Department reserves the right to approve all or deny any portion of an application.
- In accordance with Section C. Eligibility Information, an entity must be in qualified status as of the Application Review Date for their grant application to be considered by the state awarding agency. Application Review Date: August 12, 2021
- 2. Merit-Based Review Appeal Process
- In accordance with GATA Administrative Rules, Section 350, Merit Based Review of Grant Applications, a merit-based application review is required for competitive Grants and Cooperative Agreements, unless prohibited by State or federal statute.

- A. Competitive grant appeals are limited to the evaluation process. Evaluation scores may not be protested. Only the evaluation process is subject to appeal and shall be reviewed by IDHS' Appeal Review Officer (ARO).
- B. Submission of Appeal.
- 1. An appeal must be submitted in writing to Stephanie Pointer at Asia.Moore@illinois.gov who will send to the ARO for consideration.
- 2. An appeal must be received within 14 calendar days after the date that the grant award notice has been published.
- 3. The written appeal shall include at a minimum the following:
- the name and address of the appealing party;
- identification of the grant; and
- a statement of reasons for the appeal.
- C. Response to Appeal.
- * The DHS will acknowledge receipt of an appeal within fourteen (14) calendar days from the date the received.
- * DHS will respond to the appeal within 60 days or supply a written explanation to the appealing party as to why additional time is required.
- * The appealing party must supply any additional information requested by DHS within the time period set in the request.
- D. Resolution
- * The ARO shall make a recommendation to the Agency Head or designee as expeditiously as possible after receiving all relevant, requested information.
- * In determining the appropriate recommendation, the ARO shall consider the integrity of the competitive grant process and the impact of the recommendation on the State Agency.
- * The Agency will resolve the appeal by means of written determination.
- * The determination shall include, but not be limited to:
- * Review of the appeal;
- * Appeal determination; and
- * Rationale for the determination.

F. Award Administration Information
- 1. State Award Notices
- Applicants recommended for funding under this NOFO following the above review and selection process will receive a notice of award. The award notice shall include:
- a. The terms and conditions of the award.
- b. Specific conditions, if any, assigned to the applicant based on the fiscal and administrative (ICQ), programmatic risk assessments (PRA) and merit-based review.
- The announcement of the grant award shall be published by the awarding grantor to www.grants.illinois.gov
- A written Notice of Denial shall be sent to the applicants not receiving the award.
- 2. Administrative Requirements
- Upon the receipt of an award, the successful applicant is obligated to comply with the following:
- a. The Grant Accountability and Transparency Act (30 ILCS 708);
- b. The applicable requirements of the Uniform Guidance (2 CFR 200), and;
- c. Any additional programmatic and administrative special conditions that may be required.
- d. The agency awarded funds through this Notice of Funding Opportunity must provide services as set forth in the grant agreement and shall comply with all applicable provisions of state and federal laws and regulations pertaining to nondiscrimination, sexual harassment and equal employment opportunity including, but not limited to: The Illinois Human Rights Act (775 ILCS 5/1-101 et seq.), The Public Works Employment Discrimination Act (775 ILCS 10/1 et seq.), The United States Civil Rights Act of 1964 (as amended) (42 USC 2000a-and 2000H-6), Section 504 of the Rehabilitation Act of 1973 (29 USC 794), The Americans with Disabilities Act of 1990 (42 USC 12101 et seq.), and The Age Discrimination Act (42 USC 6101 et seq.). The applicant awarded these funds shall provide services as set forth in the DHS grant agreement and shall act in accordance with all state and federal statutes and administrative rules applicable to the provision of the services including indirect cost rate requirements in Section C - Eligibility, of the NOFO.
The legal agreement between DHS and the successful applicant(s) will be the standard DHS Uniform Grant Agreement. If selected for funding, the applicant will be provided a DHS grant agreement for signature and return.
