Once a resident leaves a treatment facility, the facility's staff cannot act as the client's authorized representative or use their SNAP benefits.
When the client leaves, the facility must:
If the client does not contact the Family Community Resource Center with their new address, send a request for the information to their last known address before they entered the facility. If the client does not respond within 10 days, take action to cancel the case.
The facility must give the Illinois Link card and:
The facility must keep a receipt showing the amount of SNAP benefits returned to the client.
