Once a resident leaves a treatment facility, the facility's staff cannot act as their authorized representative or use their
Food Assistance (SNAP/State Food) benefits.
When the customer leaves, the facility must:
- give the customer a Supplemental Nutrition Assistance Program (SNAP) Change Report Form; and
- tell the customer to report their new address to the Family Community Resource Center within 10 days; and
- notify the Family Community Resource Center (FCRC) in writing that the customer has left.
If the customer does not contact the FCRC with their new address, send a Verification Checklist to their last known address before they entered the facility. If they do not respond within 10 days, act on the information according to their reporting status, Mid-Point Reporter vs. Change Reporter.
Note: Mid-Point Reporting households are not required to report a change of address during their certification period.
The facility must return the Illinois Link card to the customer and:
- If Food Assistance benefits were issued for the month but have not been used on behalf of the customer, the facility must give the entire month's Food Assistance benefits to the customer.
- If any portion of the monthly Food Assistance benefits was used on behalf of the customer, the facility may or may not be required to give the Food Assistance benefits to the customer, as follows:
- If the customer leaves before the 16th of their fiscal month, they receive one half of their monthly Food Assistance benefits.
- If the customer leaves on or after the 16th of their fiscal month, they receive none of their monthly Food Assistance benefits.
The facility must keep a receipt showing the amount of Food Assistance benefits returned to the customer.
