WAG 05-01-01: Persons in Residential Treatment Facilities

Revised Manual TextThe list of Substance Use Prevention and Recovery (SUPR) licensed treatment facilities can be found on the Caseworker Quicklinks page of the Department of Human Services (DHS) OneNet.

Teen Challenge and Reformers Unanimous International are not listed on the Bureau of Policy Development's (BPD) quarterly listing, but they are certified by Revised Manual TextDivision of Substance Use Prevention and Recovery.

If a Family Community Resource Centers (FCRC) learns of a facility not listed on the website that are requesting New manual textFood Assistance (SNAP/State Food) benefits for their residents, the FCRC will contact the Bureau of Policy Development (BPD) by sending a completed form, Request For Specific Case Guidance (IL444-2150), to: DHS.PolicyDevelopment@illinois.gov. Do not deny them the right to apply for benefits. New Manual TextExample 1: Marissa has an active SNAP case. She has moved into ABC facility, which is a residential, inpatient treatment facility. As part of her admission process she gives possession of her Link card to the facility, which is a requirement. The FCRC is notified that Marissa is now residing in a treatment facility and reviews the listing of approved facilities in Caseworker Quicklinks to ensure ABC is licensed by SUPR, however the facility is not listed. The FCRC worker sends a Request For Specific Case Guidance (IL444-2150) to their LOA who signs and emails it to BPD to determine if the facility is certified by SUPR.

The following day, the worker receives an email from BPD verifying that the facility is an approved SUPR facility. The worker updates the Authorized Representative, Living Arrangement, and Individual Demographics (Treatment/Rehabilitation information) pages on the active case in IES and certifies the case.

New Manual TextExample 2: Adriana and her 3 children have moved into XYZ facility which is licensed by SUPR. XYZ staff member, serving as authorized representative for Adriana, applies for Food Assistance benefits for the family.

The FCRC worker verifies that the facility is listed on the approved SUPR facilities located in the Caseworker Quicklinks. All eligibility factors are verified through electronic clearances, the application is processed, SNAP is approved, and detailed case comments are completed. The worker completes an Illinois Link Card Issuance Form (IL444-3658) and mails the Link card to the facility.
