Long Term Care Nursing and Supported Living Program Facilities

Illinois State Law (Public Act 98-0651) requires all long term care Nursing Facility (NF) and Supported Living Program (SLP) facility providers to use the Application for Benefit Eligibility (ABE) system to submit applications. The paper application is returned to the facility with a request to resubmit using ABE. A paper applications will be accepted when an individual or someone applying on behalf of an individual, other than the facility, submits the application.
