Service coordination activities include:
- an ongoing assessment, if agreed to by the client;
NOTE: Even if the client agreed to an assessment at Intake, ongoing assessment is still optional.
- effective case management of assistance programs (e.g., processing REDEs timely, budgeting income appropriately, etc.) to ensure the client/family receives all the benefits to which they are entitled; and
- linking the client/family with other resources they may need both in and out of DHS.
The Family Assessment and the RSP are working documents. At each contact with the client, the worker needs to reassess the family's situation so that realistic goals and steps can be set. New or additional supportive services may be provided, if
called for, to help the client/family reach their goals.
