GA 2495 Thresholds PreCOVID-19 Capacity (510-TCVD)

Application and Submission Information

To be eligible for a grant award, all applicants must complete the Pre-Qualification Process. See Grant Application Information and Instructions webpage for more detail.

At the time of application, all applicants must submit the following documents:

  1. Uniform Application For State Grant Assistance (pdf). Applicants must use their SAM-registered legal name. Each application must be sent in a separate email. Note: Page 1 of the application is pre-populated. Applicants only need to complete pages 2 and 3. The completed application must be emailed no later than November 9, 2020, 5:00 PM to the following email address with the subject line stating: Your Organization's Name; Program; Barb Roberson (i.e., XYZ Agency, Program 520-TCVD; Barb Roberson).
  2. Programmatic Risk Assessment (PRA). See link on previous page. The PRA is to be completed and submitted in the Grantee Portal by November 9, 2020, 5:00 PM. A hard copy is not needed.
  3. CSA Uniform Grant Budget. The final budget document MUST be submitted via the IDHS CSA system ( The pdf budget or paper copy will not be accepted. The Uniform Grant Budget must be completed and submitted in the CSA Tracking System by November 9, 2020, 5:00 PM .
