  1. Program Description

    Scope of Services

    The Federal Communications Commission has assigned 988 as the new nationwide, 3-digit number for the National Suicide Prevention Lifeline (the Lifeline). The Lifeline is a network of crisis centers serving the entire country. The Lifeline's call routing is based on crisis center call capacity and availability. Beginning July 16, 2022, all phone service providers must ensure all 988 calls are routed to the Lifeline. To support the roll out of 988 and to increase in-state/local capacity to answer calls to the Lifeline, the Illinois Department of Human Services-Division of Mental Health (DHS/DMH) is seeking entities interested in becoming new Lifeline call centers. In the first quarter of 2020, over 16,000 calls were initiated from Illinois callers, with only 4,921 (31%) answered in state. By adding call centers in uncovered and underserved areas, DMH is seeking to increase the statewide capacity to answer calls in-state to a target of 85% in the first year of funding working towards 100% capacity in following years.
     
    Entities seeking funding are required to ensure sufficient levels of staffing are maintained and new staff recruitment, hiring and training is completed within the first nine months of funding, and the call center location is created meeting the minimum requirements of the Lifeline.


    Current National Suicide Prevention Lifeline call centers located in Illinois with already determined coverage areas may utilize this funding opportunity to ensure that sufficient levels of staffing are maintained, and new staff recruitment, hiring and training is completed within the first nine months of funding, and the call center location is expanded if necessary to cover the needs of the new staff, all changes to the call center must ensure the National Suicide Prevention Lifeline standards are maintained


    Deliverables/Milestones

    1. Reporting Requirements:
      1. Time Period for Required Periodic Financial Reports. Unless a different reporting requirement is specified in Exhibit G, Grantee shall submit financial reports to Grantor pursuant to Paragraph 13.1 and reports must be submitted no later than 30 days after the quarter ends.
      2. Time Period for Close-out Reports. Grantee shall submit a Close-out Report pursuant to Paragraph 13.2 and no later than 60 days after this Agreement's end of the period of performance or termination.
      3. Time Period for Required Periodic Performance Reports. Unless a different reporting requirement is specified in Exhibit G, Grantee shall submit Performance Reports to Grantor pursuant to Paragraph 14.1 and such reports must be submitted no later than 30 days after the quarter ends.
      4. Time Period for Close-out Performance Reports. Grantee agrees to submit a Close-out Performance Report, pursuant to Paragraph 14.2 and no later than 60 days after this Agreement's end of the period of performance or termination.


        Grantee shall submit a quarterly Periodic Financial Report (GOMBGATU-4002 (N-08-17)) to the appropriate email address.     2. Performance Requirements

      The Provider will:

      1. Identify a lead call center staff member that operates as the call center director/ supervisor and serves as the primary liaison with the Division of Mental Health.
      2. Engage in a contractual agreement with the National Suicide Prevention Lifeline and become a local call center with a designated primary coverage area and a back-up coverage area by county and zip code.
      3. Develop and maintain a secure call center location where staff can engage in phone conversations with Lifeline callers ensuring the protection of clients' rights consistent with the Illinois Mental Health and Developmental Disability Code and any additional expectations of the National Suicide Prevention Lifeline. The call center's phone system must meet these minimum requirements :
        1. The ability to return a true busy signal (also known as a network or carrier busy signal) once the estimated wait time reaches a predetermined length,
        2. Dual Tone Multi-Frequency (DTMF) Compatibility,
        3. Note: Centers utilizing Automatic Call Distribution (ACD) technology, which places callers into an internal queue at the call center if counselors are not immediately available, will be required to implement what Lifeline calls Active Answer procedures. With Active Answer, call counselors at the local center will need to press "1" in order to accept a Lifeline call. This process provides some assurance that Lifeline callers are not automatically being placed in long queues at local centers and will allow the Lifeline to re-route calls to backup centers if the center using an ACD system is unable to answer within a specified timeframe. DTMF compatibility is needed in order to implement Active Answer and pass Lifeline automated Quality Assurance (QA) test calls. This means that the center will need to have a phone system that is capable of sending DTMF tones or keypad tones to be technologically compatible with the Active Answer and automated test call features. Centers that do not have DTMF tones will not be able to implement Active Answer or pass the QA test calls and will need to work with their phone providers to modify their current telephony systems or will not be eligible to participate.
      4. Develop and maintain policies and procedures to ensure the successful operations of the local call center, including policies and procedures for providing referrals and access to resources for local callers as well as calls outside of the center's primary coverage area. Referrals and resources should be consistent with the cultural and linguistic needs of callers including veteran appropriate resources.
      5. Include active or new staff members that at a minimum meet the MHP definition and are representative of the local population, ensuring the cultural and linguistic responsiveness of the crisis services provided.
      6. Develop and maintain a staffing plan that includes 24/7 staffing sufficient to respond to the call volume anticipated by the Lifeline, and a dedicated individual to answer only Lifeline calls for coverage areas; must meet a minimum of 85% answer rate during the first 9 months of the program working towards an answer rate of 100%.
      7. Develop and maintain training for call center staff, including the principles and components of the Lifeline's Suicide Risk Assessment Standards and Policy for Assisting Callers at Imminent Risk.
      8. Participate in any required National Suicide Prevention Lifeline network evaluation activities to promote quality assurance for network operations (e.g., call logs).
      9. Develop and implement a continuous quality improvement process that includes, at a minimum, ongoing call monitoring of Lifeline calls to ensure quality. 5 % of all calls received by crisis counselors must be silently monitored monthly and all crisis counselors given feedback accordingly.
      10. Develop and implement a supervisory process that ensures all call responders have access to a supervisor at all times.
      11. Develop and implement a process for callers to provide feedback to the call center and to report concerns and grievances.
      12. Develop and implement a process for callers to provide feedback to the call center and to report concerns and grievances.
      13. Provide monthly data to DMH including, but not limited to:
        1. Call volume and answer rates
        2. Average speed to answer calls
        3. Caller disposition categories
        4. Number of calls by suicide experience categories (i.e. loss survivor, attempted survivor, suicide attempt in progress, suicide ideation etc.)
        5. Number of calls that resulted in emergency dispatch (911, CARES, etc.)
        6. Number by category of how callers learned about the Lifeline

    3. Payment

      Reference the Uniform Grant Agreement, Article IV Payment, Section 4.2 Return of Grant Funds and 4.3 Cash Management Improvement Act of 1990. Payment will be issued monthly and reconciled with reported allowable expenses.

  2. Funding Information

    This NOFO is considered a competitive application for funding. It is not a guarantee of funding.

     
    This award utilizes state appropriated funds.

    Applicants must submit a program plan which supports the level of funding and details service delivery and deliverables.

    1. Funding Restrictions 

      IDHS/DMH is not obligated to reimburse applicants for expenses or services incurred prior to the complete and final execution of the grant agreement and filing with the Illinois Office of the Comptroller.

    2. Allowable Costs

      Allowable costs are those that are necessary, reasonable and permissible under the law and can be found in 2 CFR 200 - Subpart E - Cost Principles.

    3. Unallowable Costs

    4. Indirect Cost Rate Requirements

      Please refer to 2 CFR 200.414 regarding Indirect (F&A) Costs. To charge indirect costs to a grant, agencies must have an annually negotiated indirect cost rate agreement (NICRA). If the agency has multiple NICRAs, DHS will accept only the lesser rate. There are three types of NICRAs:

      1. Federally Negotiated Rate;
      2. State Negotiated Rate; and
      3. De Minimis Rate
    5. Renewals

      This program will be awarded as a 6-month term agreement with 2, one-year renewal options. Renewals are at the sole discretion of the IDHS and are contingent on meeting the following criteria:

      1. Applicant has performed satisfactorily during the most recent past funding period;
      2. All required reports have been submitted on time, unless a written exception has been provided by the Division;
      3. No outstanding issues are present (i.e. in good standing with all pre-qualification requirements); and
      4. Funding for the budget year has been appropriated in the state's approved fiscal year budget
  3. Eligibility Information

    This funding opportunity is not limited to those who currently have an award from the IDHS.

    1. Registration and Pre-Qualification

      Prior to applying for any Notice of Funding Opportunity (NOFO), every applicant must first be registered and prequalified through the following steps. This must be done on or before the application's due date or the application CANNOT be accepted for review. 

      1. Apply for or update your DUNS number. This must be done yearly.
      2. Apply for or update your SAM registration and receive a SAM cage code. This must be done yearly.
      3. Be registered and in good standing with the Illinois Secretary of State. (This is not required of governmental entities and schools).
      4. Register with the GATA/CSFA system at https://grants.illinois.gov/registration/.
      5. Not be on the Federal Excluded Parties List;
      6. Not be on the Illinois Stop Payment list;
      7. Not be on the Department of Healthcare and Family Services Provider Sanctions List;
      8. Register and access both the IDHS Community Service Agreement (CSA) tracking system and the Centralized Repository Vault (CRV).
    2. Indirect Cost Rate 

      In order to charge indirect costs to a grant, the applicant organization must have an annually negotiated indirect cost rate agreement (NICRA). There are three types of NICRAs: 

      1. Federally Negotiated Rate. Organizations that receive direct federal funding, may have an indirect cost rate that was negotiated with the Federal Cognizant Agency. Illinois will accept the federally negotiated rate.
      2. State Negotiated Rate. The organization must negotiate an indirect cost rate with the State of Illinois if they do not have a Federally Negotiated Rate. Or the organization may elect to use the De Minimis Rate. If an organization has not previously established in indirect cost rate, an indirect cost rate proposal must be submitted to the State of Illinois through the indirect cost rate system, CARS, no later than three months after the effective date of the award. If an organization previously established an indirect cost rate, the organization must annually submit a new indirect cost proposal through CARS within six months after the close of the grantee's fiscal year. All grantees must complete an indirect cost rate negotiation or elect the De Minimis Rate in CARS to claim indirect costs. Indirect costs claimed without an established negotiated rate or a De Minimis Rate election in CARS may be subject to disallowance.
      3. De Minimis Rate. An organization that has never received a Federally Negotiated Rate may elect a de minimis rate of 10% of modified total direct cost (MTDC). Once established, the de minimis rate may be used indefinitely. The State of Illinois must verify the calculation of the MTDC annually in order to accept the de minimis rate.

    3. Content and Form of Application Submission

      Each applicant is required to submit a Uniform Application for State Grant Assistance. This is a 3- page document with the first page already completed by the Division of Mental Health. This document must be signed and dated.

    4. Program Proposal
      1. The Program Proposal shall not exceed 10 pages. Items included as Attachments are NOT included in the page limitations.
      2. The Program Proposal, including attachments should be sequentially page numbered.
      3. All documents must be typed using 12-point type,100% magnification and use black typeface on a white background, Except for letterhead and stationery for letter(s) of support.
      4. The Program Proposal must be typed single-spaced with 1-inch margins on all sides. 
      5. The PDF submission must be on 8 1/2 x 11-inch page size
      6. The submission shall include the 3-page Uniform Application,10-page Program Proposal and attachments. These must be in PDF format and submitted as one document.
    5. Budget Requirements

      A budget and budget narrative need to be completed in the CSA tracking database. There is space when preparing the budget on each line item for the budget narrative. Instructions for the CSA Tracking System can be found at https://www.dhs.state.il.us/page.aspx?item=61069.

      A Budget Template and Instructions can be used as a tool to assist in determining expenses; however, the final budget must be completed in the CSA Tracking System database. The pdf budget or paper copy will not be accepted nor should be included in the application packet.

      The budget narrative should describe how the specified resources and personnel have been allocated for services and activities described in the budget.

      The budget should be prepared to reflect 6 months of funding.

      1.       Grantees must complete the Fiscal and Administrative Risk Assessment in the GATA/CSFA system, also known as the ICQ (short for Internal Controls Questionnaire). Be sure to click "submit" to submit your answers when complete. This is done only once per entity per fiscal year. While it does not have to be completed prior to submitting the application, this step must be done before an applicant or their application can be considered.

    9. Programmatic Risk Assessment (PRA)

      Potential grantees must submit a Programmatic Risk Assessment for each grant they apply. This must be submitted before an award can be made. 

    10. Agreement Terms

      The term of the agreement will be January 1, 2021 continuing through June 30, 2021 and will require the mutual consent of both parties, be dependent upon the Grantee's performance and adherence to program requirements and the availability of funds. IDHS may withdraw this Notice of Funding Opportunity at any time prior to the actual time a fully executed agreement is filed with the State of Illinois Comptroller's Office.

    11. Application Review Information

      All applicants must demonstrate that the NOFO requirements are met as identified in Section C.

    12. Program Proposal Criteria

      The maximum possible score is 50 points. All submissions will be reviewed, evaluated and based on the Criteria listed below.

      Need - Description of Need The purpose of this section is for the applicant to provide a clear and accurate picture of the need for these services within the community and how the proposed project will address these needs. To successfully respond to this section, the applicant will need to provide demographic information regarding the identified coverage area, including the anticipated number of calls they expect to respond to. Provide any information available on the suicide rate by demographic for the area you are proposing to serve. 5
      Quality - Description of Program Services The purpose of this section is for the applicant to provide a detailed, clear and accurate picture of its intended program design. This should include the staffing plan demonstrating the required supervisory structure. Include your organizations plan for setting up the physical requirements of the call center within 9 months, a plan for responding to Lifeline calls, and the staffing plan that will meet the 24/7/365 requirement, and training requirements. 15
      Data Collection, Evaluation and Reporting To ensure accountability at all levels of service provision, IDHS is implementing the practice of performance-based contracting with its Grantee agencies. The articulation and achievement of measurable outcomes help to ensure that we are carrying out the most effective programming possible. At a minimum, Grantees will be expected to collect and report data indicators and measures as described in this NOFO. Information in this section should demonstrate an understanding of the required data reports required by both the Lifeline and DMH. The applicant will also need to describe a Quality Assurance process that meets the requirements of the NOFO. 5
      Resource Availability Describe what resources and other knowledge, skill and abilities in addition to those specific to the duration of the funding cycle the applicant possesses or will budget for in order to support the objective of this program. Information in this section should describe how the proposed program, staffing, and training plans will be funded and the applicant's current staff that have the knowledge and skills necessary to implement the program. 10
    13. Review and Selection Process

      Proposals will be reviewed by IDHS/DMH staff familiar with the requirements of the program including services to be performed in specified geographic location, if applicable. Review team members will have no conflicts of interest and will read and evaluate proposals independently.

    14. Merit-Based Evaluation Appeal Process

      Competitive program grant appeals are limited to the merit-based evaluation process only. Evaluation scores cannot be protested. Only the evaluation process is subject to appeal.

      An appeal must be submitted electronically, in accordance with the grant application document.

      An appeal must be received within 14 calendar days after the date that the grant award notice has been published.

      The written appeal shall include at a minimum the following:

      1. the name and address of the appealing party;
      2. identification of the grant;
      3. a statement of reasons for the appeal

    State Award Notices

    1. Anticipated Announcement and State Award Dates  

      It is anticipated that Notices of State Award (NOSA) will be made in December 2020.


      Applicants recommended for funding under this NOFO following the above review and selection process will receive a NOSA. The NOSA shall include:

      1. The terms and conditions of the award.
      2. Specific conditions assigned to the grantee based on the potential grantee answers on the Fiscal and Administrative Risk Assessment (ICQ), the Programmatic Risk Assessment and the Merit-Based Review.


      The NOSA is not an authorization to begin performance or incur costs. The NOSA is a notice of the State's intention to make an award but should not be construed as a guarantee of award. A grant award is not considered to be fully executed until both parties have signed the grant agreement.


      After acceptance of the NOSA, announcement of the grant award shall be published by the awarding agency at www.grants.Illinois.gov. The grant agreement will also be published in the CSA Tracking System for signature.


      A written Notice of Non-Selection shall be sent to the applicants not receiving awards.

  6. Administrative and National Policy Requirements
    1. Applicants awarded these funds shall provide services as set forth in the IDHS grant agreement and shall act in accordance with all state and federal statutes and administrative rules applicable to the provision of the services including indirect cost rate requirements in Section B: Funding Information, #4 Indirect Cost Rate Requirements.
    2. The legal agreement between IDHS and the successful applicant(s) will be the standard IDHS Uniform Grant Agreement.
