Required FFATA Reporting Notification

All organizations funded by SUPR in the CSA Tracking System are required to complete their FFATA information for their organization under the "Provider Info" tab. To review the GATA Budget Instruction Manual (under the Help tab in the CSA Tracking System) please visit https://csa.dhs.illinois.gov/gtpsecure/gtp. Section 3.2 of the Manual (pages 8-12, also attached for reference) has the location and instructions for completing the information.

The due date for this information to be completed is Friday, September 11, 2020.

Below are 3 questions that must be completed:

Question 1:  In your business or organization's previous fiscal year, did your business or organization (including parent organization, all branches and affiliates worldwide) receive (1) 80% or more of your annual gross revenues in U.S. federal contracts, subcontracts, loans, grants, subgrants and/or cooperative agreements AND (2) $25,000,000 or more in annual gross revenue from U.S federal contracts, subcontracts, loans, grants, subgrants and/or cooperative agreements?

If your organization did not receive $25,000,000 or more, answer No and leave Questions 2 and 3 blank. If there was $25,000,000 or more last year, and 80% or more was from Federal funds, select Yes and continue to question 2.

Do not fill out Q2 if Q1 was answered No.

Question 2:  Does the public have access to information about the compensation of the senior executives in your business or organization (including parent organization, all branches and all affiliates worldwide) through periodic reports filed under section 13(a) or 15(d) of the Security Exchange Act of 1934 (5 U.S.C. 78m(a), 78o(d)) or section 6104 of the Internal Revenue code of 1986 (i.e., on IRS Form 990)?

If you answer Yes, leave Q3 blank. If you answer No, then you must complete Q3.

Do Not fill out Q3 unless Q1 was Yes (Last Year was at least $25,000,000 and at least 80% was Federal) and Q2 was No. Public does not have access to information about the compensation of the senior executives.)
