2466 - GA - COVID-19 Community Garden Food Desert Pilot Project

To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire, Programmatic Risk Assessment and Uniform Grant Application. See Grant Application Information and Instructions webpage for more detail.

At the time of application, all Bureau grantees must submit for each grant program the following documents:

*Uniform Grant Application (pdf)

*CSA Budget/Tracking System and Registration Information

The completed documents are to be emailed by August 12, 2020 to the Bureau email address Dan.Blair@illinois.gov  with the subject line stating:

*Last 4 digits of CSFA #2466 Provider Organization Name - For example, 2466 ABC Help agency or 2466 We Care Center
