WAG 01-01-04-a: Use of Case Records

For TANF text deleted and AABD cases, when information is released for any purpose, document in the case record the following:

  • the name of the person requesting the information;
  • the agency represented;
  • the date of the request;
  • the date of the release of the information;
  • the specific information released; and
  • the reason for the release.

Make an entry in the case record when any request for case information is denied. Explain the reason for denial.

A Family Community Resource Center administrator may, if they desire, require supervisory approval of the decision to approve or deny a request to release case information.

Use Consent to Release Information (Form 34) to obtain an applicant's or client's permission before releasing any information.
