  • Summaries of quarterly calls with DHS. These minutes highlight important announcements, clarifications, and upcoming changes.

Reporting & Forms

• WIC Vendor Incident Reporting Form
  • Use this form to report suspected violations, issues, or incidents that affect the WIC Program.
• WIC Vendor Change Notification Form
  • Report changes in ownership, location, hours, formula supplier, or to request voluntary withdrawal. (Note: withdrawal cannot be used to avoid sanctions.)
• UPC Submission Request
  • Instructions and form for requesting that a product UPC be reviewed for inclusion on the Authorized Product List (APL).
