This Information Bulletin notifies all CILA providers of the implementation for an electronic version of the Health Risk Screening tool.
Health Risk Screening Tool:
The DDD is implementing the electronic version of the Health Risk Screening Tool (HRST) effective July 1, 2019. This is to comply with Public Act 100-1111 and passed and signed into law. DDD has contracted with HRS Inc. the company which holds the patents and copyright license for the paper and electronic HRST.
The cost for the electronic or paper version of the HRST is $3.00 per month per person or $36.00 annually per person. The $3.00 per month payment to utilize the HRST is being required by the copyright and patent holder regardless of whether your agency continues to utilize the paper version or switches to the electronic version of the HRST. Additional and detailed information about the electronic HRST will be forthcoming at a future date.
Additional funding will be incorporated to the "Miscellaneous Consultants" in each person's CILA rate to cover the monthly cost of the HRST. The DDD will incorporate the additional funding for the HRST with the FY20 COLA. With the unexpected delay in implementing the FY20 COLA the DDD will begin funding the additional cost for the HRST retroactive to July 1, 2019 with an increased HRST reimbursement when the FY20 COLA is implemented. The DDD will adjust funding for the HRST as necessary in the event the COLA cannot become effective until after July 1, 2019. The table below shows the funding which will be added to each person's CILA rate depending when the COLA becomes effective.

There is also a one-time "start-up" cost with HRS, Inc. the company which holds the copyright and patents to the HRST. The DDD has contracted with HRS Inc. to cover the $500.00 per CILA agency one-time start-up, initiation and training fees for CILA agencies to sign up and enroll with this entity to utilize the electronic version of the HRST. Additional and detailed information about CILA provider enrollment will be forthcoming at a future date. The DDD covering the initial provider enrollment cost for the electronic HRST is a one-time funding opportunity and is only being offered to the CILA agencies which participate in the initial enrollment for electronic HRSTs. CILA agencies which request a waiver to use paper HRST forms or begin operations after this initial enrollment will have to fund the initial enrollment cost.
