NOFO – 710 - Outreach

One new Division of Mental Health Notice of Funding Opportunities (NOFO) for State FY 2019 has been re-posted on the DMH Webpage at for program 710, Outreach.

For program 710, Outreach, applicants must follow all instructions listed in the NOFO.

Section C, #1 and #2 "Eligibility Information" criteria of the NOFO has been revised for Williams and Colbert Consent Decrees.

Williams Consent Decree: Applicants must be a nationally recognized peer and family operated 501C3 organization; the organization's site base must be physically located within the boundaries of the city of Chicago, with capability to travel to the physical sites of the 24 SMHRFs that are located outside of the city of Chicago - the larger Cook County, Lake County, Kankakee County and the cities of Peoria and Decatur, as needed; the organization must have the ability to hire, train and supervise person with lived experience to work as Outreach staff.

Colbert Consent Decree: The applicant must apply for and be awarded contracts for program 780, Transition Coordination and program 730 Quality Administrator. The applicant must also be Medicaid certified to provide ACT and/or CST services and have ACT and/or CST services in place. The site must be physically located within Cook County and serve the west and northwest side of Chicago and the near West and near Northwest suburbs such as Norridge, Melrose Park, Franklin Park, Schiller Park, Harwood Heights, Bellwood and Maywood. The applicant must complete the Colbert Grant Application.

The effective dates to apply are 4/27/2018 through 5/3/2018. Deadline is 5:00 pm CST.
