Summary:
The Supplemental Nutrition Assistance Program (SNAP) CHANGE REPORT FORM is completed by the SNAP customer to report any changes to his or her SNAP case. Form 1978 (English and Spanish) is available in the Forms Library on OneNet.
Form 1978 (English and Spanish) does not replace the From 1979 (English and Spanish) which is used by FCRC staff when a client reports a change to the FCRC regarding his or her SNAP, Cash or Medical cases.
Changes may be reported by the client (PM 18-04-01):
- in person;
- by phone or by mail; or
- on a change report form.
