27-60-01: Assistance Unit Line

Item 60 is the first in a series of items (60 through 74, 78, and 79) that provides detailed information on each person listed on the Form 552. The assistance unit line can include:

  •  members of a Cash, Medical, or FS Only case; and
  •  component FS Members; and
  •  a Representative Payee.

When there is not enough space on the first Form 552 for all the persons, use a 2nd Form 552 and note this fact in REMARKS. Fill in only Items 1, 5, 8, 9, 10, 60 through 74, 78, and 79 on the 2nd Form 552.

Members of the Cash, Medical, or FS Only Case

Members of a Cash or Medical case are listed in the following order: first adult (code 1 in Item 69), 2nd adult (code 4 or code 2 in Item 69), and then the children (oldest child first and others in descending order of age). For FS Only cases, persons are listed in age order, oldest first. Complete Items 60 through 74, 78, and 79 according to the specific item instructions.

Component FS Members

List persons who are on the Form 552 for FS only, following the members of the Cash or Medical case. List them in order of age, oldest first. On the turnaround Form 552, the members who receive only FS, are listed below a blank assistance unit line which begins with 3 asterisks (*). Complete Items 60 through 74 according to the specific item instructions. For Item 78, Type of Benefit, enter code 7 or 8. (See WAG 27-78-00.)

If the person receives cash or medical in another case, enter the cross-reference case number in Item 60.

Representative Payee

A payee identified by an indicator of RPY, RPN, or TEMP in Item 8 must be listed on an assistance unit line.

The only information required for a payee is their name. You may request the person's birthdate and Social Security Number. However, if the information is unavailable or the person refuses to supply it, do not deny or cancel the case. In addition to the Item 60 information, enter 25 in Item 62 and an X in Item 78.

When entering RPY information (name, birthdate, RIN, SSN, and other available information), see the instructions for the specific item.

If the payee is not included in the case, the turnaround Form 552 lists the payee below a blank assistance unit line which begins with 3 asterisks (*) and follows the unit members. When there are members of a Cash or Medical case and component FS members, the turnaround Form 552 lists a payee who receives no benefits below a 2nd blank assistance unit line which begins with 3 asterisks (*).
