Use Voluntary Acknowledgment of Paternity (Form 3416B), when the mother and alleged father are willing to acknowledge paternity of a child born out-of-wedlock or out of a civil union in the State of Illinois, other U.S. state, territory, possession, or commonwealth.
A mother and father of a child born out-of-wedlock can establish paternity by signing Form 3416B and having it properly witnessed.
If the mother comes to the interview and gives you Form 3416B, already signed by both parents and properly witnessed, paternity is established.
If the mother and the father come to the interview and sign Form 3416B in front of you, witness the signature with your full name, and use the Family Community Resource Center address. Be sure the question regarding the mother's marital status at the time of conception or birth is answered appropriately. When Form 3416B is signed and properly witnessed, paternity is established.
A parent or guardian's signature on this form is not needed if the mother and/or the father is a minor. Accept the minor's signature.
