If suitable for a different Provider, a new referral must be made.
  • Within 10 calendar days of being notified by the Office of Workforce Development, the FCRC sends SNAP Employment and Training (SNAP E&T) Program Provider Determination Notice (Form IL444-4974) to inform the volunteer that they are no longer fit to participate with their current Provider and if they wish to continue their participation in SNAP E&T they must contact their FCRC to request a new referral.
