WAG 14-06-04-b: Proof of Exemption from the Requirement

PM 14-06-04-b.

Minor Parent Currently or Previously Married

  1. (FCRC) Verify marital status by a marriage certificate, divorce decree, newspaper clipping, wedding invitation or announcement, Verification of Birth, Marriage, or Death (Form 97), or any other evidence that shows the minor parent is or has been married.

    NOTE: If the applicant has never been married or cannot verify the marriage, show marital status as never married. 

  2. (FCRC) Document the exemption.
    1. (Intake) Complete the Minor Parent Live-At-Home screens and provide details in the case write-up.
    2. (IM) Complete Minor Parent Live-At-Home Verification (Form 3604).

Parent/Legal Guardian Deceased or Whereabouts are Unknown

  1. (FCRC) Verify the parent/guardian's death by a death certificate, funeral card, newspaper clipping, Form 97, medical records, or other evidence, including a signed statement from someone in a position to know.
    1. When using a signed statement from someone in a position to know, the statement must include the date and place of death and the verifier's name, address, and phone number, and their association with the deceased.
  2. (FCRC) Document the exemption.
    1. (Intake) Complete the Minor Parent Live-At-Home screens and provide details in the case write-up.
    2. (IM) Complete Minor Parent Live-At-Home Verification (Form 3604).

Parent/Guardian Will Not Allow the Minor Parent and Child(ren) to Live With Them

  1. (FCRC) Obtain the parent/guardian's name, address, and phone number from the client.
  2. (FCRC) Attempt to contact the parent/guardian to verify the client's statement.
    1. If the person cannot be reached by phone, attempt to contact them by mail.
  3. (FCRC) If the person does not respond, ask the client to provide a written statement from someone who is aware of the family situation.
  4. (FCRC) Document the exemption.
    1. (Intake) Complete the Minor Parent Live-At-Home screens and provide details in the case write-up.
    2. (IM) Complete Minor Parent Live-At-Home Verification (Form 3604).

Minor Parent Has Lived Apart from Parent/Guardian for One Year

  1. (FCRC) Obtain details of the minor parent's living arrangement during the one year period.
  2. (FCRC) Verify the information using rent receipts, utility bills, school records, medical records, the written statement of other persons with whom the client lived, or any other available evidence.
  3. (FCRC) Document the exemption.
    1. (Intake)  Complete the Minor Parent Live-At-Home screens and provide details in the case write-up.
    2. (IM) Complete Minor Parent Live-At-Home Verification (Form 3604).

Physical or Emotional Health or Safety Would be in Danger

  1. (FCRC) Verify the minor parent's statement. Use reports from police, DCFS, or other agencies, newspaper clippings, or court records that show the parent/guardian may be involved in illegal activities, statements from medical or social agency personnel, or other person in a position to know the client's situation.
  2. (FCRC) Accept the client's statement if no evidence can be obtained.
  3. (FCRC) Document the exemption.
    1. (Intake) Complete the Minor Parent Live-At-Home screens and provide details in the case write-up.
    2. (IM) Complete Minor Parent Live-At-Home Verification (Form 3604).

Parent/guardian Lives Out-of-state, in an Institution, or is a Substance Abuser

  1. (FCRC) Verify the client's statement.

    NOTE: Institution means any residential facility, such as a prison, mental health center, or nursing home. 

  2. (Client) Provides the person's name and location and any evidence they may have to verify the statement.

    NOTE: A signed statement from friends or relatives may be accepted as evidence. 

  3. (Client) Document the exemption.
    1. (Intake) Complete the Minor Parent Live-At-Home screens and provide details in the case write-up.
    2. (IM) Complete Minor Parent Live-At-Home Verification (Form 3604).

Minor Parent's Return to the Home Would Violate the Lease or Health and Safety Standards

  1. (FCRC) Obtain the parent/guardian's name, address, and phone number from the client.
  2. (FCRC) Contact the parent/guardian to verify the client's statement.
  3. (FCRC) Allow the exemption if the person corroborates the client's statement or cannot be reached.
  4. (FCRC) Document the exemption.
    1. (Intake) Complete the Minor Parent Live-At-Home screens and provide details in the case write-up.
    2. (IM) Complete Minor Parent Live-At-Home Verification (Form 3604).

Minor Parent in Substance Abuse Treatment Program

  1. (FCRC) Verify the minor parent's statement by contacting the substance abuse treatment center or hospital.
  2. (FCRC) Document the exemption.
    1. (Intake) Complete the Minor Parent Live-At-Home screens and provide details in the case write-up.
    2. (IM) Complete Minor Parent Live-At-Home Verification (Form 3604).
