The client must complete a written request on the Child Care Application (Form 3455) or on any other written correspondence from the client requesting child care. The client may submit the original written request or submit a fax of the written request.
Example 1: A client sends in a fax of a handwritten note saying she wants child care benefits due to her employment. The note is signed 03/14 and received on 03/15. The client is contacted and comes into the office on 04/05. She submits a completed Form 3455 on that date.
The client has had child care costs since January. If eligible, child care can be authorized beginning 03/14, since 03/14 is later than 7 days prior to the receipt of the fax.
Example 2: A client comes into the office on 04/05 and submits a completed Form 3455 on that date.
The client's child care will begin in 3 days (04/08). If eligible, child care can be authorized beginning 04/08, since 04/08 is when the need for child care begins.
Example 3: A client mails in a Form 3455 that is signed 04/11. The form is received in the Family Community Resource Center on 04/24. The client has had child care costs since March.
If eligible, child care can be authorized beginning 04/17, since 04/17 is 7 days prior to the receipt date of the application and is later than 04/11.
