PM 06-10-04

(FCRC/All Kids Unit) Register the application through mail-in subsystem.

Item 1 - Enter the complete ID number. The case category is always 94.
Item 2 - Enter the date (MMDDYY) from the Application Date in Section III of Healthy Start Application (Medicaid Presumptive Eligibility) (Form 3089).
Item 3 - Enter the TA:
  • 01 - New Application
  • 02 - Reapplication
Items 5 & 6 - Complete.
Items 8 - 10 - Complete.
Item 33 - Enter TAR 44.

NOTE: Use TAR 46 to reopen an MPE application.
