Good Cause Exemption from Cooperation With CSE Program

Pregnancy
For a TANF adult-only pregnant woman case, require a signed statement from an approved Medicaid provider which states the woman is pregnant and the expected date of delivery. If the woman does not have a medical provider, see WAG-04-01-07-b.
Use Verification of Pregnancy (Form 2304) if a statement from an approved Medicaid provider is not presented.
