PM 02-04-05: Applications Received in the Wrong Office

If a central office unit or the All Kids Unit receives a mail-in or faxed application that should have been filed in the FCRC and that has at least a name, address, and signature, send it to the office that serves the address. Send the application as soon as possible, but no later than the next workday after the day it is received. If the application does not have a name, address, and signature, return it to the person by the close of business of the same workday. Give the applicant the location of the correct office and tell them what still needs to be completed.

NOTE: If a complete application was received centrally via fax, fax it to the correct Family Community Resource Center.

If an FCRC receives an application for medical only from an AKAA, forward the application to the All Kids Unit. Send the application as soon as possible, but no later than the next workday after the day it is received.
