Client is to send a written request for delay of hearing directly to BAH. If FCRC receives such a written request from the client:

1. Immediately send the request to BAH at 401 South Clinton, 6th floor, Chicago, IL 60607, Fax: 312 793-8573.
2. File a copy in the case record.
