Complete the attached user agreement and fax it to 1.866.265.7811 or scan and email to: firstname.lastname@example.org. After it is completed and submitted, you will receive a call from Susan Woodyard from Social Serve (ILHousingSearch.org software vendor) Community Relations Manager. She will get you set-up with a username and password and answer any questions you may have about the system.
You should have your client sign a general agency release form that allows information to be shared with the State of Illinois PAIR Online Waiting List.

The ILHousingSearch.org User Agreement which must be read, signed and returned in order to receive a username and password for the Caseworker Portal (details below).

An Presumptive Release PAIR document which shows you the language giving information release permission when you choose to continue the application process after the initial pre-screening. You should have your client sign a general agency release form that allows information to be shared with the State of Illinois PAIR Online Waiting List.
And a document that walks you through finding the Area Median Income (AMI) in your community. The PAIR online waiting list tool will automatically calculate whether or not your client is financially eligible using the income data that you enter at pre-screening.
